Who can be referred?

At Home-Start Gosport & Fareham, we aim to operate an open referral policy ensuring that support is available to any family who is experiencing difficulty and stress, and who has at least one preschool child. Sometimes funding restrictions does not always make this possible and as a consequence the availability of our service can change from year to year. Who can make referrals?

Families can be referred to us by any agency, eg. Health visitor, social worker, family support outreach worker, CPN etc. Families can also self refer to Home-Start Gosport & Fareham. We only refuse referrals when there is a lack of resources.

How do I make a referral?

A referral form should be completed for each referral. Before sending in a completed referral form the family must be made aware of the referral and its content, and consent given by the family by signing the form. All referrals received will be assessed to determine the following:

  • The referral is eligible to receive Home-Start support.
  • Does the referrer need to provide further information prior to an assessment by a Home-Start co-ordinator.
  • Does the scheme have the resources to support the family.

What happens if a referral is refused?

If the referral does not fall within the remit of Home-Start or there are insufficient resources to support the family, the referral will be informed that we cannot help on this occasion and the referral form will be sent back.

What happens if a referral is accepted?

Referrals that are accepted for support are given an initial visit by a co-ordinator before support is offered to clarify their needs and the support we can offer.

Do you have a waiting list?

Home-Start Gosport & Fareham aims to provide support to families as soon as possible.  The majority of families are visited within three months of referral, however on occasion families may be waiting longer than this due to the training of volunteers.

How will I be kept informed of the progress of the referral?

Referrers will be informed when the referral is accepted or rejected.
Referrers will receive a letter when the initial visit has been completed and when a volunteer is matched.
When reviews take place.
When support ends.
